Personal history statement, dated 28 Jan 70, prepared by CPL Pham Công Dan, reveals that he was born on 30 Oct 46 in Đông Sếp Hamlet, Hoàn Sơn Village, Tiên Soh District, Ha Bac Province, ((NVN)). He was admitted to the Youth Group on 31 Jun 64 ((sic)), enlisted on 28 May 67, and began infiltrating SVN on 17 Aug 67. Arriving in SVN, he was reassigned to D79 Battalion on an unspecified date; and to C1, D1 ((possibly aka the 1st Company, 1st Signal Battalion, VC Military Region 5)) in Apr 69.
