Item 2: (2 pages, handwritten and printed; B-3) Personal history statement, dated 25 Oct 68, prepared by Luu Dinh Giau, PVT in C94 ((unidentified)) reveals that he was born on 28 Jul 51 at Nghi Tien Village, Nghi Loc District, Nghe An Province ((NVN)). Giau enlisted in C40-Group 22 ((possibly the 40th Co, 22nd NVA Training Regt, MR IV)) on 28 Aug 67, and infiltrated SVN in Oct 67. He was WIA on 22 Jun 68 while operating in western areas of Quang Tri Province and returned to NVN for convalescence on 25 May 68 with Group 580. Giau underwent treatment at Dispensary 94 of Ha Tinh Province ((NVN)). Annotation indicates that Giau became part of a Group on 4 Dec 68 while stationed in Nhu Xuan District, Thanh Hoa Province. The document was authenticated on 28 Oct 68 by Nguyen Van Giao, CO of C94, and Tran Cong Chung, CO of D19. The red seal on the document reads: "19th Bn Group ((158)) - PAVN."
